MUSEUM CARD SIGN UP

Our Constituent Card provides you with the opportunity for first hand trips to Chicago's museums, zoos, gardens, and aquariums. With over 15 million visitors per year, these remarkable institutions offer visitors priceless opportunities to explore and enjoy the arts, sciences, nature, and culture.

We ask that only constituents of Representative Avelar request the card. If you are unsure if you live in the 85th District, please click here to check.

Please request the museum card at least two weeks in advance of the date you would like to attend. This will allow time for our office to communicate with the institutions and clarify any errors that may arise.

Please list the specific date that you are looking to visit said location. (Ex. July 3rd, 2021) Important note: We can only grant access to the card for up to TWO CONSECUTIVE days. We require at least 2 weeks notice for the request to be fulfilled.
